Criteria:
For students with physical disabilities (major, long-term impairment that severely restricts activities), who will be enrolling in the first year of a full-time program at a recognized Ontario post-secondary institution; must be an Ontario resident; a Canadian citizen or a permanent resident; demonstrate outstanding scholastic achievement, motivation, and initiative.
Checklist of items to include:
Complete application form; photocopy of proof of citizenship; photocopy of Social Insurance Number card; copy of secondary and any postsecondary transcripts; letter on school letterhead outlining midterm marks; typed, one-page letter outlining qualifications for the award, including scholastic achievement, motivation, and initiative. Three reference reports are also required: an academic reference from secondary school principal; a personal reference from an individual who is familiar with your extracurricular activities and outstanding characteristics; medical assessment from a qualified medical doctor in which your ability to undertake the proposed study program is indicated.
