CO-OPERATIVE EDUCATION DEPARTMENT

PROBATIONARY CONTRACT

This contract is a formal and final agreement between the cooperative education student, the placement supervisor, and the cooperative education teacher. Failure of the cooperative education student to improve training station learning and performance in the areas specified will result in immediate expulsion from the cooperative education placement and program. After discussion with all parties involved it has been determined that the student is largely responsible for his/her unacceptable training station performance.

The student will:

  1. Attend every scheduled co-operative education class except for reasons accepted by the teacher.
  2. Be present at the training station during the required hours and will inform both the training station supervisor and the teacher of any times that he/she will be absent due to illness or any other reason acceptable to the teacher and the supervisor.

Violation of any of the above conditions will result in the withdrawal of the student from the co-operative education program and subsequent loss of credits.
