WEEKLY TIME SHEET & JOURNAL

LPS students are required to make daily entries in each of the four areas of the Weekly Activity Report. In addition, LPS students are to record the total amount of time spent on LPS related activities each day. Students should include additional time spent on LPS related activities such as marking at home. LPS students are required to use full sentences with proper grammar and punctuation for focus area entries. LPS students will use the following points to assist them in making accurate and thorough entries in each of the four focus areas on the Weekly Activity Report.

FOUR AREAS OF FOCUS

ACADEMIC OBJECTIVES
What are the learning objectives for the students in this particular class?
Be very specific - describe the work assigned to the student that you are assisting.

STUDENT ATTITUDE/INVOLVEMENT
How are the students responding to your assistance?
If no particular student is assigned for a given class, assess the general tone of the class.

PERSONAL INSTRUCTION PERFORMANCE
What did you do during class time? Be very specific.
Describe your responsibilities for each class.
Whom did you help? How did you perform?

FUTURE STRATEGIES/COMMENTS
What can be done next time to improve the current situation?
What went wrong? What else could you try?
The key is to attempt to find ways to improve your role.
Provide an overview of the class deportment
Describe specific incidents which occurred in class.
Write a note to the subject teacher or the LPS administrator

At the conclusion of your daily entries, remember to record the "LPS Hours". List the time in HOURS:MINUTES for clarity and consistency. At the end of the week, be sure to total the time spent on LPS related responsibilities. Time on field trips and extra work done for the teacher can be added to the classroom time for any day.

SUBMISSION OF WEEKLY ACTIVITY REPORTS

Submit a Weekly Activity Report on Monday for the preceding week.
Reports will be evaluated and recorded by the LPS teacher. Subject teachers should read and sign the Weekly Activity Reports to ensure that they are accurate and complete in terms of information provided and hours recorded.
Marked reports will be filed in the Co-operative Education Office and will be accessible to students at any time.
Additional blank copies of the Weekly Activity Report will be available from the Co-op Office.
A penalty of 20% per day will be assessed to late submissions.
Weekly Activity Reports will not be accepted until signed by the subject teacher.
